The State of Illinois requires that an authorization form be completed and signed by the employee or potential employee before a motor vehicle report can be requested. Click here to obtain a printable copy of the MVR Request Authorization Form - (Adobe Acrobat Reader required).

You may complete the Motor Vehicle request form online by following the instructions below. The Horton Group will process your request upon receipt of the signed authorization forms. Please fax the signed authorization form to us at (708) 845-3001. (An authorization form is required for each MVR requested)
