Following 3 ignition events, which included 1 death and 1 injury, Western Enterprises has issued a recall of certain OxyTOTE products on aluminum cylinders. One of the ignition events occurred after a cylinder fell from its racking and the other two occurred after filling. The remediation process will include an o-ring change and cleaning.
